The Role...

Based in Cairns, you'll work directly with NDIS Participants, their families and carers, formal and informal networks, other service providers and referring agencies. Your main aim will be to support the Participant to connect with and maintain a variety of relationships which will enable them to implement their NDIS Plan and achieve their goals and aspirations through our specialist Case Management model of support.

We have on offer a Permanent Full-time role and are seeking a Support Coordinator who is able to work within a clinically-robust, evidence-based and specialist case management model, underpinned by the following values:

About You...

  • Tertiary degree in Social Work, Psychology, Human Services or related discipline

  • Highly demonstrable knowledgeable of the NDIS

  • Minimum of three (3) years’ experience in case management and/ or case conferencing within the disability sector, health or associated human welfare sectors

  • Highly developed adaptable and person-centred communication skills

  • Strong networks with human services organisations (upstream and downstream) within the identified region

  • Ability to travel (if required)

  • Demonstrated intermediate ability to use computer IT systems including MS Office and electronic Client Management and/or HR Systems

  • Queensland Provisional Green or above, Driver's Licence

  • Ability to obtain Positive Notice in the legislatively required personal history checks, specifically a Blue and Yellow Card.

  • It is beneficial, but not a requirement for candidates to have knowledge of the NDIS and the NDIA Portal, and experience in the Disability Services industry.
